2013 May - Continuous Administration of Enteral Lipid- and Protein-Rich Nutrition Limits Inflammation in a Human Endotoxemia Model

Lubbers, Tim; Kox, Matthijs; de Haan, Jacco-Juri; Greve, Jan Willem; Pompe, Jan C.; Ramakers, Bart P.; Pickkers, Peter; Buurman, Wim A.; Critical Care Medicine. 41(5):1258-1265, May 2013.

Objective: An overzealous inflammatory response is an important cause of morbidity and mortality in surgical, trauma, and critically ill patients. Enteral administration of lipid-rich nutrition was previously shown to attenuate inflammation and reduce organ damage via a cholecystokinin-1 receptor–mediated vagovagal reflex in animal studies. The current preclinical study investigates the immunomodulatory potential of a custom-made enteral nutrition during systemic inflammation in man.

2013 May - Extracorporeal Cardiopulmonary Resuscitation for Patients With Out-of-Hospital Cardiac Arrest of Cardiac Origin: A Propensity-Matched Study and Predictor Analysis

Maekawa, Kunihiko; Tanno, Katsutoshi; Hase, Mamoru; Mori, Kazuhisa; Asai, Yasufumi; Critical Care Medicine. 41(5):1186-1196, May 2013.

Objective: Encouraging results of extracorporeal cardiopulmonary resuscitation for patients with refractory cardiac arrest have been shown. However, the independent impact on the neurologic outcome remains unknown in the out-of-hospital population. Our objective was to compare the neurologic outcome following extracorporeal cardiopulmonary resuscitation and conventional cardiopulmonary resuscitation and determine potential predictors that can identify candidates for extracorporeal cardiopulmonary resuscitation among patients with out-of-hospital cardiac arrest of cardiac origin.

2013 May - Fast-track failure after cardiac surgery: external model validation and implications to ICU bed utilization

Lee A, Zhu F, Underwood MJ, Gomersall CD.; Crit Care Med. 2013 May;41(5):1205-13.

OBJECTIVE: To facilitate the planning of perioperative care pathways, a fast-track failure prediction model has been developed in patients undergoing cardiac surgery. This study externally validated such a fast-track failure risk prediction model and determined the potential clinical consequences to ICU bed utilization.

2013 May - High-Osmolarity Saline in Neurocritical Care: Systematic Review and Meta-Analysis

Lazaridis, Christos; Neyens, Ron; Bodle, Jeffrey; DeSantis, Stacia M.; Critical Care Medicine. 41(5):1353-1360, May 2013.

Background and Purpose: Intracranial hypertension and cerebral edema are known contributors to secondary brain injury and to poor neurologic outcomes. Small volume solutions of exceedingly high osmolarity, such as 23.4% saline, have been used for the management of intracranial hypertension crises and as a measure to prevent or reverse acute brain tissue shifts. We conducted a systematic literature review on the use of 23.4% saline in neurocritically ill patients and a meta-analysis of the effect of 23.4% saline on intracranial pressure reduction.
